Can Tinnitus Cause Depression And Anxiety

Will I Get Depression If I Suffer From Tinnitus

In part, cause and effect is difficult to pin down because major depressive disorder can develop for a large number of reasons. Tinnitus can also occur for a number of reasons. In most cases, tinnitus manifests as a ringing or buzzing in your ears. At times, the sound changes , but the root idea is the same. Noise damage over a long period of time is usually the cause of chronic tinnitus that is probably permanent.

But there can be more acute causes for chronic tinnitus. Traumatic brain injuries, for example, have been known to cause long lasting ringing in the ears. And tinnitus can happen sometimes with no recognizable cause.

So will you develop depression if you have chronic tinnitus? The answer is a difficult one to predict because of the wide variety of causes for tinnitus. But what seems fairly clear is that if you leave your tinnitus untreated, your chances will probably increase. The reason might be the following:

  • You might wind up socially separating yourself because the ringing and buzzing causes you to have difficulty with social communication.
  • For many individuals it can be an aggravating and exhausting undertaking to try and cope with the sounds of tinnitus that wont go away.
  • Tinnitus can make doing certain things you take pleasure in, such as reading, challenging.

Hearing Loss And Tinnitus

The mean PTAs showed no differences among the five groups or between ears . The average hearing loss was around 38dBHL among all the groups . A bivariate plot of the PTAs and the THI scores was performed and indicated that both the PTAs and THI scores displayed substantial variability, and the comparison of means demonstrated that self-assessed handicap between the groups was independent of auditory thresholds .

The pure-tone average of both ears among tinnitus patients with and without anxiety and depression.

Association Of Tinnitus With Anxiety/depression

When comparing anxiety against depression, Granjeiro et al. showed that patients with depression had milder tinnitus symptoms while patients with anxiety had more moderate tinnitus symptoms, and patient with both anxiety and depression together had the most severe tinnitus symptoms . The same study also showed that higher anxiety and depression scores correlated with higher tinnitus severity scores . In addition, our study showed that anxiety has higher correlation to more severe tinnitus than depression, consisting with Granjeiro et al.’s study . However, our study showed that patients with both anxiety and depression do not exhibit worsening tinnitus symptoms as compared to patients with anxiety or depression alone, which was different from the above study .

Our results showed that there were no differences in the mean PTAs among the five groups or in between ears in all the groups. The average hearing loss was around 38dBHL. The lack of association between the PTA results and anxiety or depression suggests that these variables are independent in our veteran population. This finding was consistent with the study of Gomaa et al., demonstrating that hearing loss is not the dominant cause of anxiety/depression .

What To Do To Relieve Tinnitus

There are a lot of things that you can do to relieve tinnitus. Here are just a few:

  • Sound therapy: The most common way of overcoming tinnitus is by masking the disruptive sound. Tinnitus sufferers used different sounds to mask the tinnitus sound. White noise machines also offer relief. Smartphone users can find apps that even allow you to create customized sound palettes to counteract your specific tinnitus noises.
  • Meditation: The idea behind meditation is a brain retraining. The therapy works by preparing the mind to consider tinnitus and come to terms with that. The method is the opposite of conventional methods that seek to prevent the tinnitus problem altogether.
  • Get your hearing checked. Tinnitus is often the first symptom of hearing loss. For some people, tinnitus could be the creating sounds missing from your auditory system due to a hearing loss. If you have a hearing impairment that can be treated with hearing aids, several options have built-in tinnitus relief. These are ideal for people who are suffering from any of these problems, and for many patients, they have had great results. If you havent checked your hearing yet, a hearing aid might be the most effective method for alleviating your tinnitus symptoms.

Concerns About Tinnitus (Ringing In The Ears) And Anxiety

Many of us have a ringing or a buzzing in our ears from time to time, often after exposure to a loud noise. Tinnitus is that same kind of ringing or buzzing, but it never goes away. It usually accompanies hearing loss. The more severe the hearing loss, in general, the more severe the tinnitus. Many people learn to manage it or ignore itbut for a small percentage, it becomes debilitating.

Tinnitus can affect job performance, result in insomnia, and, as Sergei Kochkin, the executive director of the Better Hearing Institute wrote in a 2011 report accompanying a survey on tinnitus sufferers, it can contribute to psychological disorders, such as depression, suicide ideation, post-traumaticstress disorder, anxiety and anger. The constancy of tinnitus and the perceived lack of control can provoke fear, which exacerbates the problem, leading to an ever-increasing cycle of distress. A quarter of the respondents in Kochkins survey reported that their tinnitus was disabling or nearly disabling.

The American Tinnitus Association estimates that 40 to 50 million Americans have tinnitus, and that two million are so debilitated by it that they cant function on a normal day-to-day basis.

There is a minimum amount of counseling. Full results of the trial will be available at the end of the year but preliminary data analyses suggest that including counseling makes a measurable difference, Dr. Henry says.

Mechanisms Associated With Anxiety/depression And Tinnitus

Functional MRI have shown increased signals in the middle and superior frontal gyri, the cingulate gyrus, the precuneus, and the parietal cortices in tinnitus patients, verified by EEG and MEG studies. The nonauditory structures are evidenced to play the greatest role in understanding the reaction to tinnitus, the subjective reporting of tinnitus severity, persistence of tinnitus perception, and physiological relationship to comorbid psychological factors such as anxiety and depression. Functional imaging studies show that the subgenual anterior cingulate cortex plays an important role in both coping styles and depression . The most recent study by Vanneste et al. showed that tinnitus patients using a maladaptive coping style show increased scores on the BDI and THI and experienced louder sounds and more distress in comparison to tinnitus patients using adaptive coping styles . The sgACC and ventromedial prefrontal cortex are considered central dysfunction nodes in depression. The dorsolateral prefrontal cortex has been found to play an important role in anxiety . These evidences further suggest why tinnitus can be associated with major depression, anxiety, and other psychosomatic and/or psychological disturbances.

Pathophysiology And Risk Factors

It is important to note that tinnitus is a symptom and not a disease therefore reflecting one or more underlying abnormalities. These abnormalities range from impaction of wax in the external auditory canal to acoustic tumours which will require medical or surgical attention.

A number of risk factors have been associated with tinnitus and they include increasing age, hearing loss and exposure to loud noise . Exposure to noise is the largest attributed aetiological factor in tinnitus . People can develop hearing loss and tinnitus when exposed to loud military, industrial and recreational noise. Military personnel are commonly exposed to high levels of sound. Tinnitus is the most common military service related disability amongst veterans of the United States of America returning from Iraq and Afghanistan . These events can be classed under Post Traumatic Stress Disorders in which tinnitus in these individuals may serve as a constant reminder of a traumatic event such as exposure to a blast.

Environmental sound is of importance for the awareness of tinnitus with 94 percent of individuals experiencing transient tinnitus in completely silent surroundings . Tinnitus has been suggested to be an early symptom of hearing loss particularly noise induced hearing loss but then there are studies to dispute this statement . However, tinnitus is not always secondary to hearing loss. There are individuals with normal hearing who experience tinnitus.

Facts About Tinnitus And Suicide

Dont assume that tinnitus will push you over the edge. A large Swedish study did find an increased risk of suicidebut only among women with severe tinnitus that had not been diagnosed .

Tinnitus is the most common disability related to military service, and suicide rates among U.S. veterans have risen. But in a 10-year study of nearly 8 million U.S. veterans, the suicide rate was actually lower among tinnitus patients.

You also might hear that everyone with tinnitus is depressed, but thats not true. About 15 percent of the veterans receiving care in the 10-year VA study reported tinnitus, a figure similar to what we see in other populations. Within that group, more than half54 percentdidnt have diagnosed depression or anxiety, while the rest had one or both conditions.

Gary Jacobson, director of the Division of Audiology at Vanderbilt University and Editor in Chief of the Journal of the American Academy of Audiology, argues that bringing up suicide frightens tinnitus patients unnecessarily. These patients are emotionally raw when we see them. The last thing they need is to be fearful, he wrote Healthy Hearing in an email.

That said, Schweitzer emphasized that there is nothing wrong with you if you do feel suicidal. If you are experiencing suicidal thoughts because of your tinnitus, you are not alone, and you are not doing it wrong or failing in some unfixable way, he said.

Anxiety Depression And Tinnitus

Tinnitus severity was assessed with the THI. The THI is a 25-item self-report questionnaire that has functional, emotional, and catastrophic subscales. shows that patients with anxiety had elevated total THI scores as compared to patients without anxiety . Patients with both anxiety and depression had elevated total THI scores as compared to patients without anxiety . However, there was no significant difference between patients with or without depression . shows that patients with either anxiety or depression had significantly increased functional THI scores . Similar to the functional subscale, patients with either anxiety or depression had significantly increased emotional THI scores ). However, there was no statistical difference among the five groups in the catastrophic THI subscale ). Interestingly, patients with both anxiety and depression do not have worsening subscale scores as compared to patients with either condition . There is no significant difference between patients with neither anxiety nor depression versus patients with no anxiety or no depression only in terms of THI and subscale scores .

Typical Causes Of Tinnitus

When medical professionals and subject matter experts discuss the causes of tinnitus, stress and anxiety is not usually on the list. Typical causes of tinnitus include:

  • Hearing loss. Age-related hearing loss and hearing loss caused by damage to the ear are both believed to be causes of tinnitus.
  • Head or neck injuries. Head and neck injuries, including cases of traumatic brain injury, can cause tinnitus. When the cause of the tinnitus is an injury, the sound and volume heard may vary more than other types of tinnitus.
  • Certain medications. Some medications can cause tinnitus. Symptoms usually go away when the medication is discontinued.
  • Blockages. Ear canal blockages, caused by ear wax, dirt, foreign objects, hair and even head congestion can cause temporary tinnitus. The sounds go away when the blockage is removed.

How To Treat Tinnitus From Anxiety

Tinnitus

Because tinnitus is such a complex disorder, there is no one stop treatment. The first step is to see a doctor and have them rule out any underlying medical conditions. Ask them if there are any treatments that they believe will work for your tinnitus based on the way you describe the symptoms.

Presumably, if your tinnitus is caused by anxiety, then curing your anxiety is the next step for preventing it from disrupting your life further. Even if tinnitus is not caused by anxiety, reducing your anxiety is important to make it easier to cope with the hearing condition.

Mathematical Model Explaining The Underlying Mechanism That Relates Hyperacusis And Depression

Linear regression analysis showed a small but statistically significant negative relationship between the Uncomfortable Loudness Levels and depression . Mediation analysis showed the indirect effects of ULLs on depression via anxiety , and via hyperacusis handicap . The total indirect effect was significant. The direct effect of the ULL on depression was not statistically significant. In summary, the relationship between ULL and depression was fully mediated via hyperacusis handicap and anxiety.

Although it has been reported that people with hyperacusis often have lower than normal ULLs in one or both ears and 42-47% of them may also suffer from depression, the mechanism that produces depression in patients with hyperacusis does not seem to be explained by reduced ULLs. Therefore, future research should focus on factors that might lead to depression in patients with hyperacusis.

Correlations Between The Degree Of Tinnitus And Anxiety/depression

Pearson correlation analysis was performed between scores for tinnitus and anxiety or depression. These are reported in Tables and . Functional THI scores had the strongest correlation with both anxiety and depression, followed by emotional scores. The data also demonstrated that anxiety has higher correlation to more severe tinnitus in terms of total THI score than depression.

Take Time For Yourself

While taking time to relax or even meditate does not cure tinnitus, it can help reduce feelings of stress and anxiety caused by tinnitus. Relaxation takes different forms for everyone. Some people choose to write down their feelings, others may paint, walk, exercise or read books.

If you meditate to relax, consider playing music or even guided meditation sounds in the background during this activity. This allows you to meditate while also dampening the sound of tinnitus.

Mathematical Model Explaining The Underlying Mechanism That Relate Tinnitus Loudness And Depression

MY EARS ARE RINGING – Anxiety Disorders and Tinnitus

Regression analysis showed a statistically significant relationship between tinnitus loudness and depression . Mediation analysis showed that the indirect effects of tinnitus loudness on depression via tinnitus handicap , via insomnia , via hyperacusis , and via anxiety . The total indirect effect was significant. The direct effect of tinnitus loudness on depression was not statistically significant. In summary, the relationship between tinnitus loudness and depression was fully mediated via tinnitus handicap, insomnia, hyperacusis, and anxiety.

This is consistent with the idea that high tinnitus loudness is associated with tinnitus handicap, hyperacusis handicap, anxiety, and insomnia, and these in turn lead to depression. The clinical implication for audiologists is that for patients who suffer from tinnitus, depressive symptoms may be alleviated if tinnitus-induced anxiety, tinnitus handicap and hyperacusis are managed adequately, even if the self-perceived tinnitus loudness remains unchanged. Past research has shown that although tinnitus loudness is only minimally reduced following various forms of tinnitus rehabilitation, effect of tinnitus on patients life, tinnitus handicap, typically improve. This improvement may be sufficient to reduce the severity of depression.

How Can Tinnitus Trigger Anxiety

The reality is that researchers are not entirely sure how anxiety triggers tinnitus, but they know that many people with anxiety can get tinnitus.

Anxiety activates the fight or flight system, which puts a lot of pressure on nerves, and increases blood flow, body heat, and more. This pressure and stress are very likely to travel up into your inner ear and lead to the tinnitus experience.

But most of these episodes are short-lived. It pops up at the height of an anxiety attack and then goes away quickly. That doesnt explain why so many people suffer long-term tinnitus. Experts firmly agree that fear is not likely to cause tinnitus. Instead, anxiety can trigger a mentality that makes us more likely to develop tinnitus.

Tinnitus Treatment With Cbt

Tinnitus is the perception of noise in the ears and can manifest in a variety of sounds such as buzzing, whirring, roaring, etc. Tinnitus can be acute or chronic and can vary in frequency, severity, and duration. While tinnitus is common , some people experience tinnitus at a debilitating level, which may impair daily functioning, relationships and work activities. These severe cases are often accompanied by an anxiety, depression or other mood difficulties. Tinnitus may make it challenging to concentrate and at times, may interfere with sleep.

Cognitive Behavioral Therapy for tinnitus addresses the cognitive, attentional, and behavioral processes that are typically observed in people with severe tinnitus. People with tinnitus tend to expend a lot of energy and attention scanning for the presence of tinnitus sounds. This attention reinforces the need to detect and protect the body from the sounds. Unfortunately, the more one scans and listens for sounds the more sounds one is likely to notice. People with severe tinnitus commonly engage in maladaptive thoughts regarding their experience of tinnitus, which can result in anxiety, depressed mood, insomnia, difficulty concentrating, hopelessness, and helplessness. Additionally, people may withdraw from their usual activities, as they misjudge how negatively the tinnitus will interfere with their enjoyment.
