While stigma may play a role in limiting hearing aid adoption, awareness, access, and affordability are likely the more important barriers. Hearing loss onset is slow and the consequences, need, and options for hearing care suffer from a severe lack of public awareness. In addition, hearing aids are currently dispensed via licensed individuals in a model that may not suit the needs of many Americans. The simple barrier of navigating the healthcare system may prevent many from accessing hearing care. Lastly, the National Academies estimate hearing aids cost, on average, $4700 per a set–a figure that includes the services provided by professionals to customize and support hearing aid use. While $4700 is the estimated average, the figure could range from approximately $1000-8000. Even on the lower end of the spectrum, this could be an unaffordable one time figure for some.
It is unlikely that any one factor is the main culprit. In fact, despite all of the attention it gets, cost is likely not the main factor. However, the combination of these factors could result in a cost-benefit calculus that is not enough to push one to obtain or pursue hearing care. These may include Americans with more mild hearing losses who only have trouble in limited situations and simply are not ready for comprehensive hearing care and hearing aids but who could still benefit from hearing technologies.
Are Hearing Aids Covered By Medicare Or Insurance
Hearing aids can be expensive and while not usually covered by insurance or by Medicare, its possible to find ones that work for you and ways to cover the costs.
Shop around for the best price. Many online retailers and big-box stores sell hearing aids at deep discounts.
Ask if the price youre paying is bundled or unbundled. Unbundling the costs of your hearing aids may eliminate line items you have no use for.

A New Class Of Hearing Aids
A hearing aid is a device worn around the ear that makes desired sounds more audible for people with hearing loss. Hearing aid devices include a microphone, amplifier and a miniature loudspeaker to make sounds louder. Traditionally, these devices have been accessible only from licensed hearing aid dispensers or audiologists.
In 2017, the FDA Reauthorization Act designated a new class of hearing aids that will be available over the counter to increase the accessibility and the affordability of hearing aids for U.S. adults. These hearing aids will be able to be purchased without a medical evaluation by a physician or a fitting by an audiologist. However, OTC hearing aids are intended only for adults who believe they have mild to moderate hearing loss.
Implementation of these regulations is a year overdue, largely because of the COVID-19 pandemic. In July 2021, President Joe Biden directed the U.S. Department of Health and Human Services to issue proposed rules within 120 days for how OTC hearing aids can be marketed and sold.
This will also get hearing aids into patients ears more quickly. It typically takes an average of four to five years after individuals recognize their hearing loss before they see a health care provider, and sometimes an additional six years to obtain a hearing aid device. With this new law, people will be able to purchase OTC hearing aids as soon as they become aware of their hearing difficulties.
When Will Otc Hearing Aids Become Available For Sale
Even with latest order issued by President Biden, the new OTC category of hearing aids will likely not be available to consumers for some time.
The practicability to act on these considerations will take effect upon the FDA releasing its final draft, commented Dr. Amlani. The final draft is expected within the next 18 months, after the completion of a public comment period and re-review of the guidelines.
There have been many calls on the FDA to release the much delayed OTC draft regulation. We hope that President Bidens Executive Order will result in definitive action by the FDA within 120 days. Once the draft regulation is released, we anticipate a public comment period, followed by further FDA review. It could take up to nine months or more for the final regulation to be released. Effectively, OTC hearings aids are not likely to be available to consumers until mid to late 2023.

Why The New Label
While an estimated 30 million Americans have hearing loss, only about one to three in 10 adults older than age 50 who might benefit from hearing aids use them. This matters because hearing loss can lead to social isolation, a decline in memory and thinking skills, and a higher risk for dementia. But hearing aids — the primary treatment for hearing loss in older adults — simply aren’t an option for many Americans.
A pair of hearing aids runs $5,600 on average — a cost that health insurance doesn’t typically pick up. In order to get the devices, people with hearing loss must have a medical exam to rule out the slim chance of a serious medical problem that is causing the hearing loss, or they must sign a waiver opting out of the exam. You can only buy hearing aids through an audiologist or a licensed hearing aid dispenser, who is authorized to test hearing and sell hearing aids.
The cost of the hearing aids covers these professional services and may include up to 4 years of follow-up at no extra cost. But hearing professionals usually contract with just a few brands. That means that choosing a hearing professional limits a person’s hearing aid choices, and changing audiologists as a result of location or personal preference could require a person to change hearing aids, too.
Due in part to these hurdles, once a person starts to lose their hearing, they wait an average of 7 years before they seek help.
Otc Hearing Devices Legislation
In 2017, legislation was passed to authorize the FDA to create a new classification of entry-level hearing devices. While final recommendations were due summer of 2020, the pandemic has delayed the authorization indefinitely. These hearing devices would be designed for individuals with perceived mild to moderate hearing loss and would be purchased over the counter. This legislation is in line with recommendations from both Presidents Council of Advisers on Science and Technology and the National Academies of Science Engineering and Medicine recommendations. The Over The Counter Hearing Aid Act plans to make starter hearing aids more accessible to Americans with hearing loss. Safety regulation and standards have not yet been determined by the FDA. Word of Caution: lack of final authorization has not prevented manufacturers from selling devices labeled as FDA-registered or FDA-cleared in an effort to circumvent government oversight.

A New Law Promises To Make Some Aids More Affordable In The Future
For years, consumers with mild to moderate hearing loss have had few options for relief. At one end of the spectrum are pricey prescription hearing aids that start at about $1,650 each. At the other end are over-the-counter alternatives, such as personal sound amplifiers hearing aid lookalikes that range in price from just $20 or $30 to about $350 each.
But PSAPs aren’t regulated by the Food and Drug Administration for effectiveness and safety as strictly as prescription hearing aids are. And manufacturers of PSAPs are not permitted to call their products hearing aids or to claim that they can help those with hearing issues to hear better. This can be confusing for consumers.
New legislation signed into law late last week, however, might even the playing field for consumers looking for effective and safe options that wont break the bank.
The legislation, introduced by Sens. Elizabeth Warren, D-Mass., and Chuck Grassley, R-Iowa, calls for the creation of an entirely new category of hearing aids. These hearing helpers will be sold OTC but will be legally considered hearing aids, and aimed at those with mild to moderate hearing loss.
Like today’s PSAPs, they will be cheaper and easier to obtain than prescription aidsbut like prescription hearing aids, will be more tightly regulated by the government.

Otc Hearing Aids Will Likely Come With Challenges
Adjusting for hearing is much more complicated than for vision, making it challenging for people to finely tune their own hearing devices.
With vision, youre looking at one variable: Is vision clear or blurry? When we test , we test every frequency, from 125 hertz to 8 kilohertz, that allows us to appreciate everything from music to speech, Kim explains.
When fitting someone for a hearing aid, you then have to set each frequencys maximum comfort level and maximum threshold level and fine-tune it for each individuals hearing loss patterns and even ear canal shape.
Beyond the complex tuning process, the process of hearing loss is typically so gradual that the brain adapts to a new reduced input. Living with this new normal means that adjusting a hearing aid to proper levels can be nearly impossible for individuals to do on their own.
When we do a good job bringing back sound across frequency and across input levels, the person will not like the sound for a week or so until the brain adapts to this new normal, Palmer says.
If you had the person do this themselves or asked them how things sounded, you would slowly tune the hearing aid to mimic the hearing loss because this is what the person considers normal until they are exposed to the correct levels of sound, Palmer states.
Difficulties self-tuning are problematic in part because individuals might think that hearing aids just arent helpful for them and stop seeking additional medical care, Kim expresses.
Overdue Otc Hearing Aid Rules
First introduced in 2016 by Senators Elizabeth Warren and Chuck Grassley , Congress ultimately passed a bill in 2017 directing the U.S. Food and Drug Administration to develop regulations for a new category of OTC hearing aids.
The OTC provision of the legislation, better known as the Over-The-Counter Hearing Aid Act, would allow hearing aids to be used by adults to compensate for mild to moderate hearing impairment to be sold over the counter. Additionally, the bill directed the FDA to issue regulations containing safety and labeling requirements for the new OTC category of hearing aids within the next three years.
However, the FDA failed to meet the 2020 deadline and has yet to issue the proposed rules this has prompted criticism from Senators Warren and Grassley, as well as consumer advocacy groups.
Inclusion of the OTC hearing aid rule within President Bidens executive order is welcome news to many who championed the legislation.

What Is An Over The Counter Hearing Aid
First off, OTC hearing devices are available today, they are known as Personal Sound Amplification Products or PSAPs. Perhaps youve seen ads on late night TV or offers for low priced hearing devices available online. These devices are currently not regulated nor can they be called a hearing aid. Prices are very low, ranging from $15 -$300 or more a piece. They are amplifiers that simply make all sounds louder, where a hearing aid can be programmed by a professional to meet the unique hearing loss of the patient. If you have a high frequency hearing loss it makes no sense to make the lower frequencies louder too. It will only make it more difficult to hear what a person is trying to say, especially if other noise is present. Since the rule making is left up to the FDA and the process is only just beginning, we have no idea what will be considered as an OTC hearing device. Any changes in classifications and new products are years away from being available for purchase.

What Are My Chances Of Being Satisfied With An Otc Hearing Aid
It’s hard to say, but most research indicates that people who’ve tried out OTC hearing aids need the help of a knowledgeable hearing care provider. A small 2017 trial provides some clues. It tested the outcome when adults aged 55 to 79 years with mild-to-moderate hearing loss chose among three pre-programmed hearing aids on their own for both ears. These were high-end digital mini-behind-the-ear aids, one of several common hearing aid styles.
As Catherine Palmer, AuD, director of Audiology at the University of Pittsburgh, notes, a large majority90 percent of participantstried more than one hearing aid. But close to three-quarters picked the wrong aids based on their audiograms. In addition, although they saw a video and received handouts, 20 percent asked for extra help using the aids.
