    Side effects that are unique to the eye must be considered when using fluoroquinolone ophthalmic solutions. Accumulation of ciprofloxacin crystals on the surface of the corneal ulcer has been noted. In all studies, precipitate resolved following treatment and in no cases did it necessitate premature discontinuation of treatment.21,24,27 Ofloxacin has not precipitated on the cornea.21,25,26 Other ocular side effects of topical fluoroquinolones include chemosis, conjunctival hyperemia, tearing and foreign-body sensation. These symptoms are generally mild, self-limited and do not require discontinuation of therapy.21,24,27 In comparative studies, fluoroquinolones caused less local discomfort than fortified solutions.2527

    Fluoroquinolones are an effective and simplified initial treatment of bacterial keratitis. Resistance patterns must be followed closely to ensure continued adequacy of this drug class as empiric coverage for this serious condition. Fluoroquinolones should not be used as first-line therapy for uncomplicated bacterial conjunctivitis.

    How And When To Use Chloramphenicol

    Chloramphenicol comes in different strengths. How you apply the medicine depends on what you’re taking it for and whether you’re using the eye drops, eye ointment or ear drops.

    If you’re using 0.5% chloramphenicol eye drops, put one drop into the affected eye every 2 hours for the first 2 days. Then every 4 hours for the next 3 days, or as your doctor advises.

    If you’re using 1% chloramphenicol eye ointment, apply it to the affected eye every 3 hours . Do this 3 to 4 times a day, or as your doctor advises.

    With more severe infections, your doctor may recommend using eye ointment at bedtime and eye drops during the day. This is because the ointment sticks to the surface of the eye and eyelid and works while you sleep. The eye drops allow you to get on with your day as normal, as they don’t affect your eyesight.

    If you’re using 5% or 10% chloramphenicol ear drops, put 3 drops into the affected ear 2 to 3 times a day, or as your doctor advises.

    To use the medicine:

    • always wash your hands before using chloramphenicol – this prevents you from passing the infection to the other eye or ear
    • remove the cap just before using your medicine and replace it as soon as you’ve finished
    • do not touch the nozzle of the bottle or tube with your fingers
    • make sure the nozzle doesn’t touch your eye or ear, or your skin
    • use a mirror to help you see what you are doing
    • tilt your head back or to the side to put drops in your eyes or ears

    Can Swimmer’s Ear Be Prevented

    You may be able to prevent swimmer’s ear by using over-the-counter acetic acid drops after you’ve finished swimming for the day. But don’t use these drops if you have ear tubes or a hole in your eardrum. Dry your ears well with a clean towel after swimming, bathing, or showering. You also can use a hair dryer on the cool setting to dry the inside of the ears.

    Keep all objects out of your ear canals including cotton swabs unless your doctor has told you it’s OK to use them.

    When To See A Doctor

    Topical fluoroquinolones are now available for use in the eye and ear. Their broad spectrum of activity includes the common eye and ear pathogens Staphylococcus aureus and Pseudomonas aeruginosa. For the treatment of acute otitis externa, these agents are as effective as previously available otic preparations. For the treatment of otitis media with tympanic membrane perforation, topical fluoroquinolones are effective and safe. These preparations are approved for use in children, and lack of ototoxicity permits prolonged administration when necessary. Topical fluoroquinolones are not appropriate for the treatment of uncomplicated conjunctivitis where narrower spectrum agents suffice they represent a simplified regimen for the treatment of bacterial keratitis . When administered topically, fluoroquinolones are well tolerated and offer convenient dosing schedules. Currently, bacterial resistance appears limited.

    Since fluoroquinolone antibiotics were introduced for systemic use in the 1980s, their indications and use have increased rapidly. Recently, topical formulations for the eye and ear have appeared, expanding the options for the treatment of certain bacterial infections.

    How Can I Treat An Eye Infection At Home

    Salt water, or saline, is one of the most effective home remedies for eye infections. Saline is similar to teardrops, which is your eyes way of naturally cleansing itself. Salt also has antimicrobial properties. Because of this, it only stands to reason that saline can treat eye infections effectively.

    How Long To Use It For

    Eye drops – use the drops until the eye appears normal and for 2 days afterwards. Do not use them for more than 5 days, unless your doctor tells you to. This is because your eyes can become more sensitive or you could get another eye infection.

    I review the swab results which are usually ready after 3-4 days. The bacteria will often be the ones covered by ciprofloxacin, but other drops may be needed, including aminoglycosides. One should bear in mind that the risks of hearing organ toxicity with any of these ear drop types is very low. An untreated infection is likely to be more risky to the hearing.

    Please seek medical attention if the discharge persists, or other symptoms develop. While this is very rare, signs of hearing organ damage include noise in the ear , dizziness and hearing loss: please stop the drops if this occurs.

Hold the dropper directly over the ear, and place the prescribed number of drops into the ear canal. To help the drops roll into the ear of an adult, hold the earlobe up and back. In children, hold the earlobe down and back. Keep the head tilted for about 2 minutes, or insert a piece of cotton if your doctor directs you to do so to keep the medication in contact with your ear canal. Repeat for the other ear if so directed. Do not rinse the dropper. Replace the cap after use.
